Emory Pediatric Neurology & The Neurophysiology Department of
Children's Healthcare of Atlanta at Egleston
Neurophysiology Services are provided by the Neurophysiology Department of the Childrens Healthcare
of Atlanta at Egleston. Testing is available in the main hospital lab and in several of the CHOA
centers throughout the metro-Atlanta area. Please request an out-patient lab site that is convenient
for your patient and obtain Patient Instructions.
Test may be ordered by any physicians by calling 404-325-6065
or by down-loading this order form, signing and returning it
by fax to the EEG lab office. The EEG LAB Fax is 404-315-2795.
The EEG Secretary will call your office to obtain precertification
information and will notify the patient about the appointment.
To schedule an appointment for an EEG and an appointment with
a Emory Pediatric Neurologist please call the Pediatric Neurology
scheduler at 404-778-2400. An EEG from Egleston that is abnormal
will often speed access to an Emory Pediatric Neurology appointment.
Please review and down-load Patient Instructions and give to your patients. Reports are available
to the ordering physician by calling 404-325-6065 and will be called, faxed and/or mailed to your
office as requested.
